Valerian Drug Interactions: Sedatives, Sleep Medications, and More

Valerian root is one of the most widely used herbal sleep aids in the world. People reach for it as a gentler alternative to prescription sleep medications, assuming that because it is a plant-derived supplement, it operates through an entirely different system. That assumption is mostly wrong. Valerian works, to a significant degree, through the same GABAergic pathway that benzodiazepines and most sedative-hypnotics exploit — which means combining it with those medications is not simply adding a gentle herb to a prescription. It is stacking two substances acting on the same receptor system.

How valerian affects the brain

Valerenic acid, the primary bioactive constituent in valerian root, inhibits GABA transaminase — the enzyme that breaks down GABA, the brain's main inhibitory neurotransmitter. By slowing GABA breakdown, valerenic acid allows inhibitory signals to accumulate, producing a calming, sedative effect. Valerian also appears to act directly on GABA-A receptors, the ion channel target of benzodiazepines, barbiturates, and alcohol.

This is mechanistically distinct from simply making someone feel relaxed. GABA-A modulation is a pharmacologically specific action with dose-dependent effects on consciousness, coordination, and respiratory drive. The fact that valerian achieves it through a botanical compound rather than a synthetic one does not make the effect qualitatively different at the receptor level.

Additive sedation with CNS depressants

Benzodiazepines — lorazepam, diazepam, clonazepam, alprazolam — bind to GABA-A receptors and enhance chloride ion flow, amplifying inhibitory signaling. The non-benzodiazepine hypnotics (zolpidem, eszopiclone) act on the same receptor through a partially overlapping binding site. Barbiturates work through the same channel. Alcohol also enhances GABAergic transmission and suppresses glutamate activity. All of these, combined with valerian, are stacking multiple substances on the same inhibitory system.

The practical consequence is additive sedation: more drowsiness, more impaired coordination, slower reaction time, and in higher doses or in vulnerable individuals, potential respiratory depression. The concern is not that valerian alone is dangerous with a glass of wine — it probably is not, for most people. The concern is the combination with prescription CNS depressants, where the pharmacodynamic effects are meaningful and where individual sensitivity varies considerably. NCCIH explicitly flags this interaction and recommends disclosing valerian use to healthcare providers when taking any medication that causes sleepiness.

The highest-risk scenarios involve people who are already on therapeutic doses of benzodiazepines or sedative-hypnotics for anxiety or insomnia, and who add valerian on the assumption that an herbal supplement is a safe add-on. In those cases, the combination can produce deeper sedation than intended, with corresponding increases in fall risk, next-day cognitive impairment, and — at higher exposures — more serious CNS depression.

Other potential interactions

Some evidence suggests valerian may affect CYP enzyme activity — early in vitro data raised the possibility of CYP3A4 inhibition. The clinical relevance of this is less well-established than the GABAergic interaction, and most pharmacological guidance focuses on the sedation concern rather than CYP-mediated drug interactions. Still, people taking medications with narrow therapeutic windows and substantial CYP3A4 dependence should mention valerian use to their prescriber, since the interaction literature is not conclusive in either direction.

Valerian has also occasionally been studied for blood pressure effects, and some sources note a theoretical interaction with antihypertensive medications. The evidence for this is thin and inconsistent; the CNS depressant interaction is the primary documented concern.
